Health
care is changing rapidly, and our participation with various
insurance plans changes from time to time. The medical plans
in which we participate may be obtained by calling the appropriate
office. In some instances, your insurance company may grant
a waiver and allow us to care for you even if we are not listed
providers. Please let us know if you need special assistance
in this regard.
To
keep our billing expenses and fees as low as possible, we ask
that all office charges be paid at the time service is rendered.
Whether you are covered by a Preferred Provider Organization
(PPO) or Health Maintenance Organization (HMO), any co-payment
is expected at the time services are provided. We accept credit
cards, cash, or checks. If you need help determining if you
are covered by a PPO or HMO, please talk with our office staff.
We will file your regular health insurance claim forms at no
charge to you. You are responsible for payment of all co-pay
and deductibles.
