During
your first appointment, we will take your medical history and
make a permanent record for you. Insurance company regulations
may require that our Specialist has a referral number before
seeing you, or your appointment must be rescheduled.
Please
bring with you any previous medical records, the name of your
personal or referring physician, the telephone number of your
physician, and your medical insurance card(s) (or have your
physician send them to our office before your first visit).
We may request your permission to transfer some of your records
to our office.
To
allow enough time to complete our records, please arrive for
your first visit to our office 15-20 minutes earlier than your
scheduled appointment time.
As many of you are aware or are afflicted with severe allergies,
more particularly to aromatic colognes, perfumes and other
fragrant personal products, we ask that you refrain from wearing
any type of fragrances or aromatic products when you visit
with us. Thank you for your thoughtful consideration of others
with these allergies.
